Your health information is important. We provide multiple ways for you to access your medical records so you can stay informed and make the best decisions about your care.

We provide two ways to access your medical records:

Medical Records & Patient Portal: 

  • To access your medical records or patient portal, please contact the location where you received care.

  • Third-party medical records requests: For worker’s compensation, attorneys, and insurance providers seeking records, please fax the request to 843-536-4003.

    • To track the status of your request, please contact BHS Connect Customer Service at 513-898-1044 or email [email protected].

  • For additional help, please contact the location where you received care for further assistance.

To access and review lab or imaging results, please start by checking your Patient Portal. If you need further assistance or have additional questions about your lab or imaging results, please submit a Contact Form below.
